Was macht der Mensch, wenn er nicht restlos ausgelastet ist? Er geht shoppen. Samstags und Sonntags am besten auf den großen Weekend-Market - für uns auf direktem Wege mit dem Skytrain erreichbar: die Wagen sind dafür ähnlich voll wie der Parkplatz. Auf dem Markt gibt es wirklich alles - und nur die Aussicht, alles noch zwei Monate durch Indien schleppen zu müssen, lässt uns z.B. an dem wirklich hübschen blauen Teeservice im praktischen Tragekorb vorbeigehen und mit ein paar Flip-Flops für Simon vorliebnehmen.

und seine Autoren natürlich auch … und zwar nicht schlecht, wie unser Swimmingpool über den Dächern von Bangkok belegen mag. Eure guten Wünsche per Kommentar oder Mail sind also in Erfüllung gegangen. Denn vor genau einer Woche haben wir Klinik bzw. Guesthouse hinter uns gelassen und sind hier eingezogen - selbst gegenüber dem beeindruckenden Klinikeinzelzimmer noch eine Steigerung. Der Aufenthalt hier bringt uns dem Nirwana zwar nicht näher, aber einmal pro Reinkarnation sollte man sich so einen Erholungsmonat schon gönnen.

Wir sind im Herzen von Bangkok - nicht weit von einer der Hauptverkehrsachsen, die ab hier Sukhumvit heißt. Geprägt wird sie vom Betonständer des Skytrains. Drunter pulst trotzdem das Leben - mit je nach Tageszeit wechselnden Straßenhändlern und Passanten. Von der Sukhumvit gehen die Sois ab (Gässle würde der Schwabe sagen) - nach Norden die ungeraden: die Soi 1 zu Simons Guesthouse (dem Tor zu Bangkoks Nachtleben und zu unserer frischen Wäsche), die Soi 3 zum Krankenhaus und in der Soi 3/1 liegt Little Arabia, wo Leute, Essen und Schriftzeichen einen in eine andere Welt versetzen - und nach Süden die geraden: wir wohnen zwischen der bürgerlichen Soi 2 (Marriott Hotel und Einkaufszentrum) und der Soi 4, in der bereits Nana, Bangkoks Rotlichtbezirk beginnt. Wer mehr über die Gegend, Drogen, Prostitution und Buddhismus wissen will, dem sei der Thriller “Bangkok Eight” von John Burdett ans Herz gelegt, den wir zufällig in der Klinikbuchhandlung entdeckt haben und der wirklich hier “ums Eck” spielt.

An der Soi 3 kann man auch ein Linienboot auf einem der Kanäle besteigen, das einen rasch an den Rand der Altstadt bringt.

Dann ein Tuk-tuk zum Chao Phraya, auf dem ebenfalls Linienboote verkehren und mit dem Sky Train zurück. Das war heute Nachmittag Thomas erster richtiger Ausflug seitdem wir wieder in Bangkok sind. Alles easy, da Simon die Tour schon mal gemacht hatte während Thomas in der Klinik war - aber immerhin. Und “spicy seafood with rice” im Straßenrestaurant gleich neben der Soi 1 geht auch wieder - nachdem Thomas etwa seit Weihnachten kein asiatisches Essen mehr geschmeckt hatte.

Wie von allen drei Einsendern richtig bemerkt, habe ich mir im Krankenhaus den SCHNURRBART ABRASIERT (und lasse ihn nachwachsen anstatt ihn mühsam mit der Klinge zu stutzen).

… is our next program point. Simon has rented a nice one-bedroom apartment close to the hospital http://www.shama.com/sukhumvit/ where we checked in today.

We will definitively postpone our flight from Bangkok to Kolkata until beginning of February and extend our visa for Thailand accordingly. Thomas will go back to the hospital as an out-patient on January 17 for an exam and a second round of blood tests (including another PCR).

If Thomas’ medical readiness is confirmed and he feels 100% fit, we still intend fly to Kolkata. If not, he will fly home instead.

Until recently, the parasites that cause Malaria were categorized into 4 subtypes. Then a fifth was discovered. To keep it simple stupid let’s call the subtypes 1-5. They can be grouped into 3 classes:
- HELL: Subtypes 1 and 2 are rare and cause a mild course of the disease. But the little devil parasites will hide in your body and come out when you don’t think of them: you got chronic disease.
- PURGATORY: The most common subtype 3 makes you really suffer from the severe course of the disease (that can include unconsciousness and seizure) but then you are dismissed until the next mosquito carrying Malaria parasites bites you.
- CLOSE SHAVE: Subtypes 4 and 5 (the new kid) are mild course and you are done but unfortunately they are as rare as 1 and 2.

How do you find out which subtype(s) caused your disease?
- The simple test is counting parasites in your blood under the microscope. Limited sensitivity because if there are too few you don’t see them anymore: you reached the detection limit. Depending on the experience of the observer, it may also be difficult to distinguish the subtypes by their appearance.
- The far better results yields a PCR (Polymerase Chain Reaction) which “amplifies a single or a few copies of a piece of DNA across several orders of magnitude, generating thousands to millions of copies of a particular DNA sequence” (cited from http://en.wikipedia.org/wiki/PCR ). You can imagine that sensitivity (detecting the parasites) and specificity (distinguishing them) are increased enormously if you basically have in your Petri dish tons of copies of the few parasites in your body. The disadvantage: few centers do it (one in Thailand) and it takes seven - ten days for the results to come back.

And in my case?
Right from the beginning it looked like I was lucky enough to have caught subtype 4 (plasmodium malariae) only but we are still waiting for the PCR outcome. Keep your fingers crossed.

2. THOMAS’ FRUCHTBLASE

Im Mutterleib der Schutzgöttin aller stationären Patienten herrschen ständig angenehme 24 Grad, es ist windstill und angenehm leise - während jenseits der großen Glasscheibe, die einen Blick auf die Welt da draußen ermöglicht, die Sonne erbarmungslos brennt, der Verkehr tost, Lärmpegel und Gedränge oft schier unerträglich sind. Hier aller Komfort, perfekter Service - draußen einfache Guesthouses, aufdringliche Tuk-Tuk-Fahrer. Hier die Wahl, was man frühstücken, zum Mittag- und Abendessen essen will - draußen einfachste Straßenrestaurants oder - verkäufer mit unbekannten Gerichten. Ich sehe meiner bevorstehenden Wiedergeburt schon ein bisschen mit Sorge entgegen. Wie rasch werde ich mich wieder daran gewöhnt haben?

3. UNSER WOHNZIMMER Auf ZEIT

Hier treffen wir uns zum Reden, Diskutieren, Zeitung lesen, im Internet surfen oder Fernsehen. Hier gibt es Wasser, Tee, Säfte. Im Bad nebenan kann sich Simon die von der Stadt schmutzigen Füße waschen und die Flip-Flops gleich dazu. Hier kann er jederzeit auf dem Sofa schlafen. Hier erfährt Thomas das Neueste von draußen. Hier leistet Simon ihm Gesellschaft beim Essen - am Anfang ist noch viel für ihn übriggeblieben, jetzt muss er selbst was mitbringen. Hier bearbeiten wir Fotos und erstellen den Blog. Hier ruft Heidi an, von hier aus wird Anne angeskypet. Hier ist unser Lebensmittelpunkt auf Zeit.

4. DURCH DIE SIEMENSIANERBRILLE

Bis beinahe hierher kommt man mit dem erst im letzten August eingeweihten Air-Rail-Link, an den Türschwellern Siemens in Blech wie bei der Bangkoker Metro. Ultraschalluntersuchung mit einem Acuson Sequoia 512, das die Nutzer nach eigener Aussage lieben, während sich das S2000 nebenan manchmal noch wie ein junges Zicklein benimmt. Im Zimmer schließlich eine Siemens-Mikrowelle neben einem Toshiba-Wasserkocher (ein paar Brosamen kriegen die doch immer ab).

While in my design bathroom this morning, I put the netbook on the sideboard, set the volume control to max and clicked on CCR: Susie-Q, Bad Moon Rising and my personal favorite … Long as I Can See the Light. The best thing is the small loudspeakers create the perfect 70s radio sound. For a while I felt like I was in my teens again.

I’m certainly not going to argue with Sokrates but at least temporarily “mens sana” is very well possible in a ”corpore” that still struggles a bit.

NO worries, I’m HAPPY in the hospital - seriously!

This section is dedicated to Ms. Schreier and Dr. Grauer at the Universitätsklinikum Erlangen http://www.medizin1.uk-erlangen.de/e109/e689/e701/index_ger.html, to Dr. Wadchara and Dr. Lily and the nurses and staff at the Bumrungrad International Hospital in Bangkok http://www.bumrungrad.com and - last not least - to Simon and Heidi.

The symptoms are always the same. Mostly in the evening and always without pre-warning I start shivering and trembling and feel cold despite the fact that my temperature rises rapidly. I’m bound for a dreadful night but latest the next morning the symptoms are gone. The bad news is the next event starts more or less regularly 72 hours later. My sixth “fever attack” began when the new year was just an hour old (at least over here). But then I was already taken very good care of in Bumrungrad International Hospital and now I’m quite confident that it was the last one.

The pattern is typical for Malaria. In my case it was only detected 17 days after the first event due to a combination of unfortunate circumstances. After the second event I began seeing doctors (5 visits at 4 different doctors). Three blood tests were performed in 3 different cities. Two were negative for Malaria which is hard to detect off the actual event with the simple methods available in Lao (counting parasites under the microscope). The last one didn’t check Malaria because in the meantime I had developed symptoms typical for Dengue fever (rash, joint pain). And the test was positive for Dengue fever which can only be treated symptomatically and usually takes no longer than a week for a primary infection.

At this point I called the infectious disease, travel and tropical medicine department at the Universitätsklinikum Erlangen. Ms. Schreier instantly gave me the mail address and mobile phone number of Dr. Grauer despite the fact that he was not on duty that day and the following one. I photographed my blood test results, typed up my case history, mailed everything to Dr. Grauer and left a voice mail for him. The evening was not over in Luang Prabang (where we had arrived December 26) when he called back. The evidence for Dengue Fever was so compelling that he tended to agree with the diagnosis of his Laotian colleagues. But he was also sure that a second underlying disease was a prerequisite to explain the pattern and duration of the fever - possibly a bacterial infection since Malaria had been ruled out twice. He mailed the international name of an antibiotics which I obtained from the local pharmacy and we both hoped that a week of rest in beautiful Luang Prabang with its mild climate would finally cure me.

When the next event came the following evening, Simon send a mail to Dr. Grauer who called back quickly. The conclusion was that it was time to fly to Bangkok right away. After the decision I already felt better and was ready for a round of calls with my travel health insurance. The next day all flights to Bangkok were completely booked which was kind of fortunate given the fact that we had only slept half the night. Finally on the last day of 2553/2010 we had a smooth 100 minute flight to Bangkok and the same evening Dr. Wadchara, an infectious disease expert at Bumrungrad International, gave me a call in my hospital room and confirmed that I definitely had Malaria and not Dengue fever.

The good news came yesterday morning from Dr. Lily, the physician on my ward. She told me that the parasites were no longer detectable in my blood - despite the fact that they had plenty of time to multiply. Today Dr. Wadchara confirmed they were again below the detection limit. The start of 2554/2011 has been perfect for me: getting better every day - probably won‘t last the whole year round.

I will never forget however that Simon carried the burden of taking perfect care of me for 17 up and down days and 5 dreadful nights - up to the point that he felt ill when we arrived in Bangkok. Fortunately an examination and a reliable blood test confirmed that his symptoms were purely stress-related and that he had neither Malaria nor Dengue fever - which gave him an opportunity to celebrate the new year by hanging out in the Bangkok clubs until 5 in the morning with the gang from the guesthouse http://www.soi1guesthouse.com/ located within two minutes of the hospital.

And I do know how Heidi suffered with me - in particular when she called during the worst event only to find an exhausted son and a husband in delirium. You can tell how she must have felt since she called again at 4 at night (her time) - luckily to find her boys recovering.

My present environment contributes to my well-being. In addition to world-class medical care and benchmark nursing and service, this place - selected by the insurance - resembles more a great hotel than a hospital.
